It came from the thousands of people that hospitals are required to treat for free. That money has to come from somewhere, and that somewhere is the people that they are allowed to charge.
(The Guardian, like many in the UK, think that if the UK has ‘free’ health care, then everybody ought to have free (taxpayer funded, government provided) health care.
Of course, the long lines for treatment in the UK ought to make it clear what the problem is with this system, but people getting Free Stuff aren’t known for their expertise in economics.
